Who we are:

Westcountry Case Management is an established service, providing complex case management to client of all ages living with a variety of conditions, including birth injuries, acquired brain injuries, spinal cord injuries and multiple diagnoses. We work with clients undergoing personal injury or negligence claims, and our case managers need to be able to communicate effectively within that process.

We are seeking dynamic, passionate and kind individuals with the following:

• Professional health or social qualification, with 7rs experience preferred, such as occupational therapist, social worker, nurse, physiotherapist, speech and language therapist or clinical psychologist.
• Ability to work with the WCM values of integrity, sustainability, compassion, collaboration, quality and professionalism
• Full UK driving license with the ability to travel to and work in client sown homes
• Robust clinical reasoning skills to underpin and justify decisions made
• Excellent record keeping and report writing skills
• Highly effective verbal and written communication skills
• Ability to build therapeutic and professional relationships quickly
• Ability to work autonomously and also as part of a team, taking responsibility for your own actions
• Ability to demonstrate excellent time management and co-ordinate others when working to deadlines
• Ability to apply new learning to novel situations
• Must be IT literate and have the ability to learn new systems quickly
• Strong problem-solving skills and the ability to manage difficult and at times, volatile situations

Our Case Managers work with our clients and their families to assess rehabilitation needs, report our recommendations, commission services and lead multi-disciplinary teams, providing rehabilitation and support in the community to assist clients to reach their optimum potential.

Westcountry Case Management are registered with the CQC and are rated outstanding in all 5 areas. We are also registered with the CIW for our work in Wales. We have a strong clinical leadership team and robust induction and ongoing support for case managers.

We cover the whole of the South and West of England and Wales and are particularly seeking experienced or potential case managers located in south Devon, Somerset, Wiltshire, Bristol or surrounding areas.

Salary and benefits:

• Salary of £39-45,000.00 dependent on experience

Support Worker

FEMALE SUPPORT WORKER

We are seeking a support worker to join an existing care team to provide support and facilitate the independence of our client, a schoolgirl who lives with her parents near Swansea. She has cerebral palsy, resulting in physical and learning disabilities and requiring 24/7 1:1 care.

Her care needs are complex and she requires support in managing her day to day needs in the home; assistance in undertaking activities; supporting her to attend a special needs primary school and engaging in therapies.

Some of the moving and handling tasks may require you to bend, kneel, squat or lunge and work on mats on the floor on a regular basis.

The work is varied and involves assisting the client to access the local community and its amenities, along with accompanying her on family outings and holidays.

The role may include overnight shifts at the client’s home in future (but not at present).

Previous experience and full driving licence are essential. You will be required to drive a long wheelbase wheelchair accessible vehicle.

You must be registered with OR working towards registration with Social Care Wales.

A recognised Health and Social Care qualification (NVQ/QCF Level 2 or equivalent) is desirable

Part time and bank hours available

£13.91 per hour weekdays, £14.94 per hour weekends.
Waking nights: £14.94 per hour weekdays and £15.97 per hour weekends.

The Client likes spending time with family, watching TV, playing with her noisy toys.
Going to school, youth club and Sunday school. She loves going on days out with the family and going on rides and also enjoys horse-riding, swimming, colouring, drawing and playing board games.

Registration with Social Care Wales is desired. If not registered, you will be expected to do so within the first 6 months

A recognised Health and Social Care qualification (QCF Level 2 or above ) is desirable.

At the minimum, applicants will be expected to have some experience of caring for children or Adults with learning, physical and communication difficulties.

PART TIME AND BANK SUPPORT WORKERS

Required to support a 38-year-old gentleman who lives in his own property in Exeter with a 24-hour package of support (sleeping nights). The client is currently supported by a small team led by a highly competent and experienced team leader.
The client was involved in a road traffic collision. He has some residual cognitive, communication and emotional difficulties and fatigues very quickly.
His hobbies include, cycling, walking, gardening, cinema and listening to the radio. He loves being outside and likes staff to take him shopping and for walks on the beach or in the countryside. He also enjoys watching the soaps and is a keen Dr Who fan. He attends local day centres twice a week and likes to go for dinner with friends regularly. He also attends the Dawlish Garden Trust once a week where he is supported to engage in a variety of gardening and agricultural activities.
He has access to a vehicle which staff are insured to drive. This allows him to access the shops, community and leisure activities.

Previous brain injury experience in a similar role, full driving licence and own car are essential.

An NVQ Level 2 (or equivalent) in care and competent IT skills are desirable.

2 x 24 hour shifts a month, plus bank hours available. Travel costs will be paid for travel in excess of 30 minutes.
Days / times to be discussed at interview but likely to include a weekend shift with additional cover for holiday and sickness

£12.79 per hour day rate

Support Worker

Required to provide support to a 17 year old young man with cerebral palsy who lives in Pontypridd. He requires assistance with all personal care, eating / drinking (PEG fed) and all toileting needs. He is sling hoisted for all transfers and uses eye gaze. Very competently to communicate.

He is a keen football fan and enjoys following this on TV. He enjoys chatting and interacting with others (although not when the footie is on!).

You will be required to support with all daily living needs, helping to access hydrotherapy pool, carry out a physiotherapy plan and support him to access social activities. You will also need to complete all aspects of domestic support in order to keep his environment clean.

Previous experience in a similar role would be preferred
A full driving licence is essential.

You must be registered with OR working towards registration with Social Care Wales

Hours available – part time.

I am 12 years old. I have a brain injury, Autism, ADHD and a visual impairment. I live with my mum and younger brother, in Chilcompton. My mum and I are looking for someone to join my friendly and proactive support team.
I need help with personal care, social communication, attending appointments, accessing activities, doing my therapy exercises at home, accessing the community, planning and managing day to day activities and being encouraged and motivated. I sometimes need support with my emotional regulation.
My team includes my mum, my case manager, my support workers, my occupational therapist, my physiotherapist and my clinical psychologist. My team work together to help develop my skills and independence. My team will provide you with statutory and bespoke training. They will ask you for feedback on what helps me when you are supporting me. You will be part of my team.
I love gaming on my Nintendo Switch, playing in the garden on my trampoline and trike, watching Dr Who and Star Wars, listening to music, country walks, swimming and being with friends. I don’t always like doing my therapy, going to busy places, dealing with children I don’t know. I need help to manage these activities from someone with lots of energy. Good motivational skills are essential! So too is fitting in with my family’s life.

I live in a village location so you must be a car driver. I have my own car for support workers to drive on shift.
Hours of Work:
Rotas are mutually agreed in advance, with my Mum. Flexibility with working hours is very helpful.

Typical shift patterns will be weekdays from 15:00 - 19:00 and some Saturdays and Sundays from 10:00 - 17:00.

Extra hours are available during my school holidays and there is also the opportunity to support me overnight occasionally with sleep in shifts.
Holiday pay is provided on a pro rata basis.
Rates of Pay:
Monday to Friday: £15.00 per hour
Weekends: £16.50 per hour
Sleeping Night shift £ 9.50 per hour (8 hours)
